What is a urostomy and how is it different from a colostomy or ileostomy?

A urostomy (most commonly an ileal conduit) is a surgically created opening that diverts urine from the body after the bladder has been removed or bypassed. Unlike a colostomy (which diverts stool from the colon) or ileostomy (which diverts stool from the small intestine), a urostomy produces continuous urinary output — urine flows constantly rather than intermittently. This continuous flow creates a fundamentally different pouching requirement: the barrier must resist constant moisture exposure, the pouch must drain frequently rather than be discarded when full, and an anti-reflux mechanism is needed to protect the stoma from urine backwash.

What does the anti-reflux valve do clinically?

When a urostomy pouch is worn during normal daily activity, movement can cause urine already collected in the pouch to slosh back upward toward the stoma — a phenomenon called reflux. If urine repeatedly contacts the stoma surface and the surrounding peristomal skin, it increases the risk of ascending urinary tract infections (bacteria traveling upward into the urinary conduit) and contributes to peristomal skin breakdown from constant moisture exposure. The anti-reflux valve is a one-way mechanism inside the pouch at the stoma opening that allows urine to flow down into the pouch but prevents it from flowing back toward the stoma. This is a clinically significant urostomy-specific feature not found in colostomy or ileostomy pouches.

What does "extra-extended wear" mean for a urostomy barrier?

Standard wear barriers are designed for 1–3 day wear. Extended wear barriers are designed for 3–5 days. Extra-extended wear (Coloplast's designation for this product) is designed for longer wear periods — typically 5–7 days for appropriate patients. The key challenge for urostomy barriers is that urine is a constant, dilute acid that steadily erodes hydrocolloid barrier materials over time. Extra-extended wear formulations use adhesive materials with higher moisture resistance and slower erosion rates, maintaining their seal longer under continuous urine exposure than standard barriers. Actual wear time varies by individual output, activity level, and peristomal skin condition.

How does the spiral adhesive differ from a standard flat hydrocolloid barrier?

Coloplast's spiral adhesive structures the barrier material in a spiral pattern combining two functional properties: secure mechanical adhesion to the peristomal skin, and moisture absorption that draws perspiration and surface urine away from the peristomal skin. The moisture absorption is particularly important for urostomy patients because the stoma is in continuous contact with urine — the spiral structure helps manage this moisture at the skin interface while maintaining adhesive function. A flat hydrocolloid barrier absorbs moisture less efficiently and saturates more quickly, which can reduce wear time and increase the risk of peristomal dermatitis from prolonged moisture exposure.

What are the five adapters for?

The five adapters included in each box are nighttime drainage connectors. During sleep, a urostomy pouch needs to be connected to a bedside drainage bag so the patient does not need to get up multiple times to empty the pouch. The adapter connects the soft drain outlet of the urostomy pouch to the tubing of a standard leg bag or bedside drainage bag. Using a nighttime drainage system also prevents the pouch from overfilling during sleep, which can stress the barrier seal and increase leakage risk. Always confirm the adapter is compatible with your nighttime drainage bag before first use.

How do I choose between 265ml and 375ml volume?

Volume selection depends on output volume, emptying preference, and activity level. Most adult urostomy patients produce approximately 1,000–1,500ml of urine per day. With a 265ml pouch, most patients need to empty 4–6 times daily; with a 375ml pouch, 3–4 times. For patients who want to empty less frequently, prefer a less visible profile while maintaining a larger reserve (such as during longer outings), or have high urine output, the 375ml is the more practical choice. For patients who prefer a lower-profile pouch under clothing and are comfortable emptying more frequently, the 265ml is appropriate. The midi 9-1/2" length is the same for both.

Should I choose transparent or opaque film?

Transparent film (SKU 12472, 12474) allows visual monitoring of urine color, output volume, and stoma appearance without removing the pouch. This is clinically preferred during the early post-operative period and for patients who monitor their urine for blood, sediment, or color changes as part of ongoing health monitoring. Opaque film (SKU 12471) provides privacy and discretion — the pouch contents are not visible through clothing or during changes. Many patients transition to opaque film once they are comfortable with their urostomy and no longer need to monitor output visually at every emptying.

A Personally Challengiing Read
Don’t expect Rosaria Butterfield to coddle sin. She acknowledges the Biblical truth that we are at war with it. It is evil. Lies are rampant with it. The Bible’s solution is to kill it, nailing it to the cross of Christ. If we don’t, it will do great and lasting harm. That is the background premise of this book. This book is for believers in Jesus Christ, especially those who identify as Evangelicals. Others may benefit by reading its pages, but I suspect that most of them will not be able to persevere through it because today’s culture is just not willing to sit through this much proclamation of Bible truth. They will react, not respond. Believers will be convicted, encouraged, and edified. Butterfield’s style is plain, but today’s culture requires her to address and to use terminology that may be challenging to many readers. It was for me. Add a level of abstraction for some challenging spiritual truths. Add the fact that she is an intellectual dealing with cultural and theological subjects. It is mostly college level reading, except for the story parts. Butterfield has credentials. A professor of English at Syracuse University, she was ten years in the Lesbian community with a mate. She was found by Christ, began to grow in Him, was discipled by faithful church women, met a wonderful man, was married to him--a Presbyterian pastor—became a mother, and began to minister to college students from a Christian perspective. She tells some of her story and the stories of others in the pages of the book. It is fascinating reading. The book is organized around the five lies that Butterfield has chosen to expose, with two to four chapters dealing with each. She acknowledges that there are others that could be exposed as well. The last one, about our culture's attitude toward modesty, was the most surprising to me. The book also has a Foreword by Kevin DeYoung, a Preface, and an Introduction. Don’t skip them. The Foreword is nothing short of a good sermon that should be shared in every church in America. In the Preface Butterfield spills the beans, revealing exactly where she stands on key topics and to whom she is addressing this volume. The Introduction is engaging and lengthy and pretty much summarizes the rest of the book. Along the way Butterfield deals with homosexuality, changing gender, male leadership in the home and in church, the bedrock importance of repentance, progressive sanctification, intersectionality, “gay Christianity” (Side A and Side B), feelings and truth, empathy and sympathy, submission of the wife, feminism, inerrancy, envy and biblical contentment, suffering, modesty and exhibitionism, and even worship. She quotes scripture, sometimes at length, and she gives copious Bible references. She also has footnotes, some of which are as interesting as the text. She names and uses or responds to contemporary authors on the topics at hand. She gives solid directions for those dealing with contemporary issues. She also has suggestions for dealing with family members beset by these matters including a question-and-answer section. Page 301. I recommend this book for all Christian leaders and for believers who want to understand contemporary challenges for the church from our modern culture.
