COURSE OVERVIEW:

Welcome to the Develop Effective NDIS Individualised Support Plans course. This program is designed to provide you with the essential knowledge and practical skills needed to create detailed and individualised support plans that meet the unique needs of individuals, particularly within the framework of the National Disability Insurance Scheme (NDIS) Act. By mastering these principles and techniques, you will be able to support individuals in achieving their goals and improving their quality of life.

We begin with an introduction to Individual Support Plans (ISP), which includes the purpose and importance of ISPs. Next, we will explore how to identify individuals who require an Individual Support Plan. Then we will outline the steps involved in working with the individual to develop their plan, ensuring their needs, preferences, and goals are central to the process. Then we will cover strategies for building rapport with individuals, fostering a supportive and respectful relationship that facilitates open communication. Finally, we will delve into the key components of an ISP in accordance with the NDIS Act.

The Personal Information Section: In this section, you will learn how to complete the personal information section of the ISP. This includes accurately gathering and recording essential personal information such as the individual's full name, date of birth, contact details, emergency contact information, and relevant medical history and current health status.

The Assessment of Needs Section: In this section, you will learn how to complete a comprehensive assessment of the individual's needs. This involves evaluating their physical, emotional, social, and cognitive needs, understanding their likes and dislikes, identifying strengths and areas requiring support, and gathering input from the individual, their family, and healthcare professionals to create a holistic assessment.

The Goals and Objectives Section: In this section, you will learn how to complete the goals and objectives section of the ISP. You will discover how to set realistic and measurable goals and objectives that align with the individual's needs and aspirations, including the formulation of both short-term and long-term goals, providing a clear roadmap for progress, and developing measurable objectives to track the individual's progress.

The Support Services and Interventions Section: In this section, you will learn how to complete the support services and interventions section of the ISP. This involves identifying and planning the necessary support services and interventions such as physical therapy, occupational therapy, counselling, and designing specific programs and activities that align with the individual's interests and abilities. You will also learn strategies to support daily living activities and promote independence.

The Health and Medical Support Section: In this section, you will learn how to complete the health and medical support section of the ISP. This includes developing a detailed medical care plan that covers medication management, scheduling medical appointments, implementing emergency protocols, understanding nutritional requirements and dietary preferences, and managing chronic conditions or disabilities to ensure comprehensive health and medical support for the individual.

The Risk Management and Safety Section: In this section, you will learn how to complete the risk management and safety section of the ISP. You will discover how to identify potential risks and implement strategies to mitigate them, develop safety measures and emergency procedures, and establish guidelines for preventing and responding to incidents or emergencies to ensure the individual's safety.

The Communication Section: In this section, you will learn how to complete the communication section of the ISP. This includes identifying the individual's preferred methods of communication, using communication aids or assistive technologies if necessary, and establishing a regular communication schedule with family, carers, and service providers. You will explore strategies to enhance and support effective communication.

The Behavioural Support Section: In this section, you will learn how to complete the behavioural support section of the ISP. You will develop strategies for managing challenging behaviours, including positive behaviour support techniques and creating plans for regular review and adjustment of behavioural interventions. This will ensure that the individual's behaviour is managed effectively and supportively, promoting their well-being and quality of life.

The Social and Recreational Activities Section: In this section, you will learn how to complete the social and recreational activities section of the ISP. You will plan opportunities for social interaction and community involvement, facilitate participation in recreational and leisure activities, and support the individual in building and maintaining relationships. Social and recreational activities are crucial for the individual's overall well-being and integration into the community.

The Education and Employment Support Section: In this section, you will learn how to complete the education and employment support section of the ISP. You will set educational goals, identify support services, provide vocational training and employment support, and develop strategies for skill enhancement and employability. Education and employment are vital for the individual's personal and professional growth, and you will gain the tools to support these areas effectively.

The Legal and Advocacy Support Section: In this section, you will learn how to complete the legal and advocacy support section of the ISP. You will ensure that individuals are informed about their legal rights and entitlements, access advocacy services, and receive support for decision-making and consent processes. Understanding and respecting legal rights is essential for empowering the individual and ensuring their autonomy.

The Review and Evaluation Section: In this section, you will learn how to complete the review and evaluation section of the ISP. You will understand the importance of regularly monitoring and reviewing the support plan, assessing progress, making necessary adjustments, involving the individual, their family, and a multidisciplinary team in the review process, and documenting any changes and updates to the plan. Regular review and evaluation are critical for ensuring the ISP remains relevant and effective.

Not durable
Color: Nude, Size: Women 9-13
Only used two of them so far. One sock lasted nearly 3 wears. The other broke before I could finish wearing it the second time. Basically useless junk.

Thick Silicone Moisturizing Socks
Color: Nude, Size: Women 9-13, Color: Nude, Size: Women 9-13
★★★★☆ Thick Silicone Moisturizing Socks I ordered these silicone moisturizing socks to try for dry heels. Unlike typical fabric spa socks, these are made entirely of soft silicone and are noticeably thicker and very stretchy. When I first took them out of the package there was a visible fold line in the silicone. It appears to be from how they were folded in the packaging rather than a defect in the material. The silicone itself feels durable and flexible. The design works like a full silicone sleeve that traps moisturizer against your skin. I applied Aquaphor first and then slid these on so the moisture stays in contact with the skin longer. One thing I noticed right away is that the silicone feels cool when you first put them on, which is actually pretty comfortable. The bottoms have a zig-zag textured pattern that gives some grip if you need to stand briefly, though these seem better for relaxing while moisturizer absorbs rather than walking around much. The ankle opening is the tightest part of the sock. It stretches, but it’s noticeably snug compared to the rest of the silicone, which helps keep them from sliding off. For sizing reference, I wear a women’s 7.5–8 shoe and the size Large fits comfortably. There’s plenty of stretch and I can wiggle my toes easily, so they should accommodate larger feet as well. They’re also easy to clean. After using them you can simply wash them with soap and water and let them dry completely before storing or using them again. Overall these appear well made with thick silicone and plenty of stretch. If you use moisturizing socks or heel treatments, these work well for keeping lotion in place while your feet absorb it. Since this set comes with two pairs for about $10, the price seems pretty reasonable for something reusable like this.

Stains easily. Tight for men's size 11 feet
Flavor Name: XL silicone socks 2 pairs, Flavor Name: XL silicone socks 2 pairs
Husband wore dark cotton socks over these gel foot covers overnight and the covers picked up color from the socks. Tried hand washing, even dabbing with a bleach soaked paper towel, but the stains remained. The covers are a bit tight for men's size 11. Edit: After a few more hand washings the stains began to fade. However one tore around the leg opening when taking it off.

Worthless. Tore on first use.
Flavor Name: XL silicone socks 2 pairs
The first time I put these on my feet, the whole toe broke right through. They were a proper fit and my feet are well pedicured. Just very poor quality. I don’t recommend.
