The medal takes a distinctive four-lobed cross form in rich gilt, the central disc carrying a beautifully rendered scene of Otgontenger rising from the steppe and upper snow in white enamel, framed by laurel branches at the base. It suspends from a bold bar mount in red and dark blue enamel with the provincial name ZAVKHAN in raised gilt lettering and the numeral 90 in a gilt roundel at the crown, the colors of the Mongolian national flag.
