The DUTCH Plus is a comprehensive dried urine and saliva test that maximizes the available information from sex and adrenal hormone production and metabolism. With easy, at-home sample collection over the course of one day, providers can gain insights into the overall diurnal pattern of free cortisol and cortisone, along with the total distribution of cortisol metabolites.

This test offers further insight by including organic acid testing, which evaluates potential nutritional deficiencies, oxidative stress, gut dysbiosis, melatonin levels, and neuroinflammation.

With the DUTCH Plus, providers also get a closer look into adrenal function with details about a patient’s cortisol awakening response (CAR) by showing cortisol's natural rise and fall within the first hour of waking.

The results include:

• Analysis of 35 different hormones: estrogen, progesterone, testosterone, DHEA-S, and cortisol along with their metabolites, plus the cortisol awakening response (CAR) 

• Graphical representation of results and embedded video tutorials

• DUTCH OATs: 8-Hydroxy-2-deoxyguanosine (8-OHdG), melatonin (6-OHMS), and nine organic acid tests (VMA, HVA, KYNA, MMA, Xanthurenate, Pyroglutamate, Indican, b-Hydroxyisovalerate, Quinolinate)

The Easiest Patient Collection

Patients collect just four or five dried urine samples over a 24-hour period. Dried samples are stable for several weeks making them convenient to ship worldwide.

Effective HRT Monitoring

DUTCH testing was designed to be optimally effective for most forms of hormone replacement therapy. Unique methods are used for improved monitoring of oral progesterone and vaginal hormones.

DUTCH vs. Saliva Testing

While the free cortisol pattern in saliva has clinical value, there is a significant missing piece to surveying a patient’s HPA-Axis function with saliva testing – measuring cortisol metabolites. To properly characterize a patient’s cortisol status, free and metabolized cortisol should be measured to avoid misleading results when cortisol clearance is abnormally high or low. Likewise with sex hormones, measuring estrogen and androgen metabolites gives a fuller picture for more precise clinical diagnosis of hormonal imbalances and HRT monitoring.

DUTCH vs. Serum Testing

While the most universally accepted testing method (due to the availability of FDA-cleared analyzers that are reliable and inexpensive), serum testing is lacking in some areas. Adrenal hormones cannot be effectively tested in serum because free cortisol cannot be tested throughout the day. There is also a lack of extensive metabolite testing (especially for cortisol and estrogens).

DUTCH vs. 24-Hour Urine Testing

There are two primary drawbacks to 24-hour urine testing of hormones. First, the collection is cumbersome, and as many as 40% of those who collect, do so in error (Tanaka, 2002). Secondly, dysfunction in the diurnal pattern of cortisol cannot be ascertained from a 24-hour collection. Some providers add saliva for daily free cortisol. DUTCH eliminates the need for two tests.

Great Lesson for Age 2 and Up
This was our day care's Book of the Month, for children age 2-4 years old. They are all learning to use manners with their peers, at the table and when addressing adults. The lesson took a few readings for the children to understand the point, but once they understood what the various characters WEREN'T doing, therefore getting a negative response from Mr Panda, and what they SHOULD be doing to receive a positive response, the lesson stuck with them. If a child forgets his manners, a hint ("How would you ask Mr Panda?") has good results. This book, while a good story to read aloud, may need some additional explanation by the adult reader at first. Mr Panda offers his friends a treat, and who doesn't like donuts?! When each animal's response is rude or selfish, Mr Panda abruptly withdraws his offer and moves on to someone else. When the final offer is made to a friend who uses his manners, he gets ALL the donuts, because as Mr Panda remarks at the end, he doesn't like donuts anyway. I don't know if this sets up unrealistic expectations, but it's a good lesson presented in an entertaining way.
